The subject-verb agreement rule for 'either...or' and 'neither... nor' says that the verb will agree with the subject that is closest to it. The proximity of the subject to the verb is the only thing that matters when it's a question of 'either...or' or 'neither ...nor'.
There option A is the correct answer as the verb 'have' agrees with the subject, 'I', which is closest to it.
Options B and D are incorrect because the verb 'has' is only used with the pronouns he, she, it or with singular nouns. It does not agree with the subject/ pronoun 'I'.
Option C is incorrect because in standard English there simply is no usage as 'I is'.
