Regular verbs are those verbs that change form in the past tense. With most regular verbs, the past simple is created simply by adding -ed. In the given sentence the verb is "survey". The past simple form of "survey" is formed by adding -ed to the end of the verb. Thus option B is correct while A is incorrect. 
Option C is incorrect as no change has been made to the verb. "Survey" is not in past simple, present tense.
Option D is incorrect as the answer lies in B.
