Answer: It will greatly surprise him if they choose him.
Explanation: When the subject of the sentence is the doer, the verb is in Active Voice. When the subject of the sentence is the receiver of the action, the verb is in Passive Voice.
Here, we will focus on both clauses. In the first clause, subject 'he' receives the action of getting surprised. On the other side, 'he' in the second clause receives the action of getting chosen. Subject or doer of the action is unknown in both the clauses, so we will write 'it' and 'they' in first and second clause respectively.
If the given sentence is changed into active voice, then the object in the active voice becomes subject in the passive voice. Word 'by' is removed. Moreover, we will use objective form of he i.e. him. Since the passive voice follows will/shall + be + 3rd form of the verb structure and is/are + 3rd form of the verb structure, active voice of first and second clause will be in Simple Future and Simple Present form respectively.
