A sentence is in the active voice when the subject performs an action.
A sentence is said to be in the passive voice when the subject receives an action.
While changing an interrogative sentence to passive voice, the subject 'who' changes to an object 'by whom'.
The
passive form of 'taught' in the simple past tense is 'was/were taught'.
This can be seen in option D, hence it's the right answer.
Option
A: The word 'by' must come before the object 'whom'. Also, 'who' is a subject pronoun and must be changed to 'whom' (object pronoun).
Option B: Similarly, 'who' needs to be changed to 'whom'.
Option C: The words 'she was taught' is in the form of an assertive sentence, whereas the given sentence is interrogative. So, it must be 'was she taught' and not 'she was taught'. The former asks 'by whom' whereas the latter asserts that she was taught.
