The basic rule of Subject-Verb agreement is that a singular subject takes a singular verb while a plural subject takes a plural verb. The verb must always agree with the subject of a sentence. 

Option B is correct. The phrase "together with her seven puppies" is a prepositional phrase. Prepositional phrases never contain a subject; thus the subject of the sentence is the third person singular Sammy. It requires a singular verb, namely, "has".
Options A and C are incorrect. In A, "have" is third person plural, not singular. There is no subject-verb agreement. In C, "had" is in past tense, whereas the question requires a present tense verb.
Option D is incorrect as the answer lies in B.
