When a subject is directly acting on the object, the sentence is written in Active voice

When the object is acted upon by the subject, the sentence is written in Passive voice.
The given sentence in past perfect tense. Therefore it is expected that the position of the past participle is just after "had", i.e. had + pat participle.
Option C is correct because the given sentence shows what the subject had done. Therefore the sentence proves to be in active form along with the verb which is identified as past participle form. 
Whereas, option A states the sentence to be active but with wrong past participle; option B contains the wrong answer identified as past participle and option D contains correct verb identified as past participle but is stated as a passive sentence. Therefore, these three options are incorrect
